Welcome to the CHAMPS Quality Improvement (QI) Tools online resource. This set of tools is intended to assist Region VIII CHCs in becoming familiar with the functions of continuous quality improvement within a health care center and implementing quality improvement activities. Resources include a glossary of terms, strategies, guidelines, flow sheets, and other documents useful in performing quality improvement activities.
Note: Many of the tools are a compilation of resources shared by health center teams that have participated in a Health Disparities Collaborative. As the QI Tools online library is a dynamic collection, CHAMPS staff welcomes new tools and resources and any comments you wish to share. Please send tools and comments to shannon@champsonline.org.
