The Community Heath Center (CHC) Tobacco Cessation Program was initially piloted in Colorado at five Community Health Center (CHC) clinic sites. The program was developed by an Advisory Group of representatives from Colorado's CHCs and a core workgroup from the Colorado Community Managed Care Network (CCMCN) with funding and support from the Colorado Clinical Guidelines Collaborative (CCGC) and the State Tobacco Education and Prevention Program (STEPP). The CHC TCP program implements evidence-based guidelines in the health care setting to ensure all nicotine-dependent patients are identified and treated. This program was designed as a system-wide, comprehensive, evidence-based Tobacco Cessation Program specifically designed for CHCs to address their populations' needs and unique attributes.
NOVEMBER 2008 PROGRAM UPDATES
Great American Smokeout is November 20, 2008.
On November 20th join health care and community partners across the country to celebrate the Great American Smokeout (GASO). Community Health Centers play key roles in supporting patients and coworkers to quit nicotine on this national holiday. Here are 5-minute ideas of ways you can support your patients and employees quit their nicotine addiction on the Great American Smokeout:
- Log on to http://www.cancer.org/docroot/subsite/greatamericans/Smokeout.asp or http://acsf2f.com/gaso/ to learn about the Great American Smokeout, order GASO materials, and download fact sheets.
- Display materials in the lobby of your community health center with information on GASO, QuitLine promotional items, self-help quitting information, and information on how to obtain free nicotine replacement therapy through the Colorado QuitLine. Go to http://www.steppitems.com/ for more materials and information on tobacco cessation resources.
- Feature an employee who has successfully quit smoking in an employee newsletter. Include quit smoking facts and the www.coquitline.org link to your employees.
- Email your colleagues a description of your efforts to integrate smoking cessation into your workflows; celebrate your progress to date and thank your colleagues for their support on the project.
- Contact your local health agency and/or tobacco coalition. Include their local GASO programming in your activities.
- Write a letter to the editor to your local newspaper about the importance of quitting nicotine on the Great American Smokeout; describe how you treat and screen community health patients for nicotine addiction. State that helping your patients quit smoking is key to their health.

CCMCN TCP Increases CHC Electronic Infrastructure to Support the Tobacco Guidelines
CCMCN is beginning work this fall with HD Consult in creating an enhanced electronic infrastructure to further promote successful implementation of the Guidelines for Tobacco Cessation and Secondhand Smoke. Tobacco template development and refinement is underway. A planning process was completed in 2006 with the help of a CHC Tobacco Cessation Program Advisory Group consisting of both clinical and non-clinical CHC staff and representatives from STEPP, CCGC, Latino State Tobacco Prevention Education Network (LSTPEN), and Community Health Association of Mountain/Plains States (CHAMPS). Major achievements during the planning process included the development and completion of a needs assessment for each CHC in Colorado, a survey of all staff regarding successful program aspects and program goals, and a comprehensive work plan for the first six months of the implementation phase of the program.
The Initial CHC Tobacco Cessation Program Goals:
- Develop and implement a comprehensive, system-based Tobacco Cessation Program in Community Health Centers (CHCs) statewide.
- Work collaboratively with existing Tobacco Cessation initiatives such as the State’s Tobacco Evaluation and Prevention Program (STEPP) and other organizations.
- Cultivate and promote approaches to Tobacco Cessation which are effective in Community Health Centers.
The Expanded CHC Tobacco Cessation Program Goals:
- To engage every CCMCN CHC organization in the guideline for Tobacco Cessation and Secondhand Smoke Exposure.
- To spread and sustain methodologies at all CHCs sites.
- To build CCMCN CHC network organization infrastructure to implement the Guidelines for Tobacco Cessation and Secondhand Smoke.
- To increase the CHC electronic infrastructure to implement the Guidelines for Tobacco Cessation and Secondhand Smoke.
- To implement Guidelines for Tobacco Cessation and Secondhand Smoke Exposure in additional provider groups.

Interesting Tobacco & Secondhand Smoke Facts
■Smokers have a blood pressure about 10 points higher than normal and their heartbeat is about 20 beats faster than in a non smoker.
■Some 2 to 5 million US children suffer from asthma; of these, about 20 percent experience more asthma attacks and more severe attacks than their fellow young asthmatics, due to secondhand smoke.
■Secondhand smoke contains more than 4,000 chemical compounds, including carbon monoxide (which poisons the human body), ammonia, formaldehyde, and other poisons. 4 of the chemicals – benzene, 2-naphthylamine, 4-aminobiphenyl, and polonium-210 are classified by the EPA as known carcinogens—cancer causing agents.
■Deaths from all types of cancer among men who smoke cigars are 34 percent higher than among nonsmokers. |
