Effective self-management allows patients to have a central role in determining their care and fosters a sense of self-responsibility for health and well-being. Collaborative self-management programs encourage dialogue and agreements between the provider and patient/family that define problems, set priorities, establish goals, create treatment plans, and solve problems.
