Job Description:
Provides Individual and Group Therapy services consistent with an IDDT model within a multi-team office environment for homeless persons with psychiatric diagnoses and who may have a co-occurring substance-related disorder, in accordance with the overall mission, values and objectives of CCH.
Essential Job Functions
1. Provides Individual and Group Therapy services consistent with an IDDT model to homeless persons who may have a co-occurring mental health and substance related disorders.
2. Conducts differential and multi-axial assessments as necessary for all clients served.
3. Monitors client progress by regular, ongoing contact with individuals receiving services and contact with collaborative service providers both external and internal to CCH.
4. Works collaboratively as part of a multi-disciplinary team, completing functions necessary to this role.
5. Communicates clinical assessments, interventions and ongoing client contact through timely, thorough record keeping, route slips, contact forms and verbal communication.
6. Works cooperatively to serve clients in a comprehensive, effective and caring way.M
7. Serves as a liaison with collaborating agencies and other teams within the organization, establishing and maintaining effective working relationships with organizations and groups providing services to our target population. This job function requires availability by cell phone during regular business hours.
8. Maintains on-going competency in the use of the EHR which includes:
a. Recording all patient visits in a timely and complete fashion.
b. Treatment planning and clinical documentation in the appropriate format.
c. Managing tasks and documents via the Inbox and Provider Acceptance Queue.
d. Maintaining a passing score in all compliance audits
9. Completes Medicaid authorizations as well as any other reporting requirements necessary to obtain, and sustain funding and provide services to individuals served.
10. Meets formally with COMPASS program manager for weekly clinical supervision, and comes actively prepared to discuss administrative and clinical issues.
11. Attends and participates in clinical and administrative meetings as deemed appropriate and necessary for the job.
12. Performs additional duties deemed necessary and appropriate by supervisor.

Other:
Education and Work Experience
1. Master’s degree in social work, counseling or similar field; Colorado licensed LCSW or LPC.
2. Current Colorado licensure as a CAC II, CACIII or LAC, or NAADAC MAC credential.
3. Experience and interest working with disenfranchised persons.
Knowledge, Skills and Abilities
1. Knowledge of homeless issues and demonstrated sensitivity to underserved populations.
2. Knowledge of mental health issues (diagnosis, treatment, symptoms, and special needs).
3. Able to prioritize multiple job responsibilities, work independently and exercise good professional judgment.
4. Able to listen effectively, synthesize and analyze information and respond creatively to challenging issues.
5. Skill in operating office equipment, including computers (including e-mail), software (e.g., Microsoft Word, Excel) and telephones.
6. Able to negotiate electronic health record system as it becomes the mechanism for all documentation.
7. Effective oral, written and interpersonal communication skills.
8. Able to function as an effective and constructive member of an interdisciplinary team.
9. Able to advocate and negotiate in an assertive but tactful manner both internally and with community agencies, to have clients’ need met.
10. Able to maintain client confidentiality.
11. Able to demonstrate a sincere desire to help people.
12. Ability to pass automobile insurance carrier's motor vehicle record investigation and criminal background investigation. |
