Since 1999, hundreds of community health centers in the United States have participated in Collaboratives focusing on improving care for specific chronic conditions. Mountain/Plains Clinical Network (MPCN) is the lead clinical network for the Health Disparities Collaboratives in the West Central Cluster (Regions VI and VIII).

HDC MISSION AND GOALS
The mission of the Health Disparities Collaborative is to 1) generate and document improved health outcomes for underserved populations; 2) transfer knowledge about how to promote positive breakthrough changes; and 3) develop infrastructure, expertise, and leadership to support and drive improved health, access, and cost outcomes. Attainment of these goals depends upon continual learning, improvement, and change.
The national goal of the program is to delay or decrease the complications of chronic diseases by excelling in patient self-management, clinical decision support, positive delivery system redesign, clinical information systems, and strong partnerships with local and community organizations.
